How to get Ratthew's key

1. Keep completing hospital shifts. Ratthew is a recurring story NPC, so the whole sequence does not happen in one visit.
2. Talk to Ratthew whenever he appears. Let each conversation finish and handle him as a regular patient. Do not reject him as an anomaly.
3. Complete five interactions. His dialogue gradually reveals his concerns about Animal Corporation, his past, and his missing twin sister, Rattina.
4. Watch the table after the fifth conversation. Ratthew leaves instead of handing you the item directly. A small key appears with a note indicating that he took it from the doctor.
5. Pick up the key before leaving. On PC, approach it and hold E when the interaction prompt appears. On another device, use the displayed interact control.

If Ratthew or the key does not appear

• Make sure you have completed the preceding Ratthew conversations; the key is tied to the fifth interaction, not simply a fixed shift number.
• Finish the current patient interaction and progress to later shifts. Ratthew's exact arrival can vary with the run.
• Do not mistake him for a hostile anomaly or end the interaction early.
• Look closely at the table or surface near where he was sitting after he leaves.
• If the item fails to spawn after the full dialogue, rejoin and continue from the saved progression. Game updates can also change quest behavior.
